Pet Application
This on-line application form is intended to indicate your interest in purchasing a pet.
I agree to the following conditions: To provide food, clean fresh water, adequate shelter and kind treatment to my adopted pet and not to sell, abandon or give away said animal to be used for experimental purposes. I will provide immediate veterinary care for the animal whenever needed, I will use a harness with the Municipal License attached to it when ever the dog leave's the house and I will obey all State and Local animal regulations.
I am at least 18 years of age and I agree to the terms of this agreement.
I hereby attest that all information provided above is truthful and accurate to the best of my knowledge.
